Summer Camp Forms
- Summer Camp Registration pdf - Preschool, Junior & Senior Camps use the same registration form.
- Required Waiver doc (please mail in with registration form) - Waiver required of all students.
- Summer Camp Health Form pdf - Summer Camp Health Form is 2 (two) pages. Verify you see both pages.
- Camp Brochure pdf (no registration form) - A copy of the brochure detailing more information about our great camp program!
- Food Allergy Action Plan pdf (Epi-Pens) - For parents of children with severe allergic reactions (nuts, bees, shellfish, etc.) to complete.
- Lunch is an option. Please contact our summer camp business office directly.
- Job Application pdf - If you want to apply for a job.
- Reference Forms pdf (for Job Applicants) - Form to list your references on.
- CIT / Volunteer Application doc - If you wish to work or volunteer at the camp.
